In the period from 1981 to 2002 we are investigated 19 children with congenital lobar emphysema – 13 boys and 6 girls. Patients age: 3 newborns (1 day – 1; 4 days – 1 and 5 days – 1), 1 month – 2 patients, 2-3 month – 3 patients, 4-6 month – 5 patients, 1-2 year – 2 patients, older – 3 patients. 11 patents had pathological findings in the right lung and 8 patents  - in the left lung. Most often emphysema was localized in the middle lobe of the right lung (8 cases) and in upper lobe of the left lung (7 cases). In 10 cases it was found concomitant pathology: congenital heart disease, neuromuscular disease, bronchopleural fistula, etc. Respiratory distress syndrome was the most frequent problem of all our patients. We found only 3 patients without clinical signs of respiratory insufficiency. Lobectomy were performed in 18 patients, 3 of them died after surgical treatment.
